Parent Contact Log - HMS

The Parent Contact Log is used to document all communication between Hudson Middle School teachers and parents/students.

The Parent Contact Log is located in a folder in your Google Drive.  Searching "Parent Contact Log" should bring this up.  If you need help finding your Parent contact Log, please email Mrs. Young.

When you access the form, please make sure that you are using the one that is maroon at the top and has only 4 questions:

1. Student Last Name
2. Student First Name
3. Date of Contact
4. Summary/details of communication

You might find it helpful to add a link to the form and/or results spreadsheet to your computer desktop.  This can be done easily by opening the form/spreadsheet, highlighting the web address and then dragging and dropping it to your desktop.
